Are you a qualified Band 4 Assistant Practitioner looking for your next challenge? Do you have a passion for delivering high-quality patient care and making a real difference in the lives of service users and their carers?

We are currently recruiting two highly motivated and compassionate Assistant Practitioners to join our dynamic and friendly Adult Community Mental Health Team (CMHT) in West Norfolk. This is a fast-paced and rewarding environment where your skills and dedication will be truly valued.

We are looking for innovative and resilient practitioners; a strong commitment to person-centred care; an ability to work collaboratively within a multidisciplinary team; and a passion for improving mental health outcomes in the community.

You would be joining a supportive and inclusive team with opportunities for professional development and a chance to contribute to meaningful change in community mental health services.


Main duties of the job

Working alongside our Registered Nurses, Social Workers and Occupational Therapists, you will support with the day to day activities and provide support to service users throughout their recovery journey. You will hold and manage an identified caseload where appropriate; take part in and contribute to ongoing review of risk assessments, recognising and responding to any changes; attend and contribute to MDT reviews; and liaise with family and carers.

Working within an experienced team, you will be supported with regular supervision and opportunities to work in partnership with health and social care in the wider multi-disciplinary service.

Person Specification


Qualifications

* Care certificate or relevant equivalent
* Foundation Degree in Health and Social Care/mental health or apprenticeship in H&SC/mental health L5


Experience

* Working with Service Users with mental health needs
* Working within Multidisciplinary Team
* Working knowledge and understanding of CPA process


Other

* Ability to travel independently


Knowledge

* Understanding of mental health issues
* CPA

£27,485 to £30,162 a yeargross per annum
