This is a new and exciting opportunity based within the memory service in Brent. In collaboration with Dementia UK we are introducing a new Admiral Nurse Service to support families affected by dementia and are looking to recruit 2 dementia specialist Admiral Nurses to set up and lead this service.
Admiral Nurses are specialist dementia nurses who offer expert practical, clinical and emotional support to families involved in the care of a person living with dementia with complex needs. Based within multi-disciplinary community teams and working alongside nurses, therapists, primary care, social care and third sector providers, amongst others, you will also benefit from being a part of a clinical network of Admiral Nurses working across London and beyond.
Dementia UK will support you to maintain and strengthen your clinical and leadership expertise as a specialist Admiral Nurse through regular clinical supervision and practice and professional development opportunities. You’ll become a welcome part of a network of Admiral Nurses and will have the opportunity to access relevant post-graduate training and develop professionally, informed by the Admiral Nurse Competency Framework.
Main duties of the job
For the Admiral Nurse clinical lead posts we are looking for skilled registered nurses (adult/mental health/learning disability) with a passion for, and significant demonstrable track record of, providing person-centred and relationship-centred dementia care with a strong sense of advocacy and equalities for this client group. Combining direct work with families with supporting the best practice of health and social care professionals, you will call upon your impressive knowledge of bio-psychosocial interventions to improve the experience and quality of life of families affected by dementia.
Please also note that in order to prepare you for becoming an Admiral Nurse, candidates are encouraged to complete the ‘Pre-Admiral Nursing Module’. Detailed job description and main responsibilities

Clinical
5. Provide clinical leadership on the care of people with dementia and their carers/families with complex care needs from peri diagnosis to post bereavement support
6. Provide specialist nursing assessment, using the Admiral Nurse Assessment Framework for carers/families affected by dementia
7. Provide a range of bio-psychosocial interventions to promote health and wellbeing for people with dementia and their carers/families
8. Work collaboratively with other professionals to improve the identification and management of co morbidities and frailty for people with dementia and their carers/families
9. Develop and support care plans for people with dementia and families including positive risk management
10. Support people with dementia, carers/families and staff to understand and respond to changes in behaviour and relationships as a result of dementia
11. Provide care coordination for a clinical caseload of families with complex needs as identified within triage assessment in clinics or via home visit if required
12. Ensure the safeguarding of vulnerable adults and that all legal requirements relating to Mental Capacity Act 2005, Mental Health Act 1983 (amended 2007), and Deprivation of Libert Safeguards (2009)[1] are considered and adhered to in relation to patient care
13. Act as an advocate for people with dementia and their carers/families to promote their rights and best interests
14. Work collaboratively with health, social and voluntary sector services to achieve coordinated and integrated care
15. Work in partnership with colleagues to promote the use of advance care planning for people with dementia and to support and inform families, around advance care planning, best interest decisions and end of life care
16. Manage and advise on complex care transitions for people with dementia and their carers/families to reduce the frequency of crisis situations
17. Ensure that care is delivered in accordance with relevant local and national guidelines and all relevant CNWL policies
18. Recognise the limits of own competency and professional boundaries and make appropriate and timely referrals to other services where required
Education and Leadership
19. Facilitate and support the development and delivery of innovative dementia training and education to staff, to improve competence and confidence, in line with best practice and guidance
20. Participate in and/or lead formal training events promoting best practice in dementia care and disseminating Admiral Nursing work
21. Provide professional and clinical leadership and mentorship, acting as a clinical role model on the delivery of evidence-based practice in dementia care within CNWL
22. Advise and support on the development and delivery of work streams as part of CNWL’s, dementia strategy, where appropriate
23. Work proactively with key local and national stakeholders to develop more integrated care pathways and holistic models of care, through dissemination of specialist skills and knowledge
24. Contribute to the delivery of local policy, procedures and protocol for CNWL to improve identification, assessment, ongoing support for people with dementia and their family carers
Evaluation and Audit
25. Support the audit and evaluation of quality improvements and initiatives aimed at improving the delivery of dementia care within CNWL, where appropriate to role
26. Collect and review quantitative and qualitative data, as agreed, to evaluate and audit the Admiral Nurse Service and make necessary improvement
27. Undertake analysis and evaluation and make any recommendations to the Brent ICP Board
28. Ensure service information and data recorded is relevant, accurate, complete and captured in a contemporaneous manner
29. Support evaluation of workforce development in dementia to improve staff knowledge and skills across the host organisation
