The Role

The Community Care Pathways Lead provides strategic and operational leadership for the coordination, implementation, and oversight of non-clinical care pathways within the Lanarkshire Healthy Lifestyle & Weight Management Service (LWMS). This is a non‑patient facing role, focused on managing community care pathways, supporting service improvement, and enabling effective systems working across the organisation.

Ensure pathways are robust, efficient, and aligned with national and local priorities, including the Population Health Framework, Diet & Healthy Weight Delivery Plan, and associated national weight management minimum standards. Working closely with clinical, operational, and corporate colleagues, the role supports integrated, person‑centred service delivery and improved outcomes across adult and paediatric pathways by ensuring consistency, sustainability, and effective delivery at scale.

Post holder will lead the coordination, implementation, and oversight of agreed pathways across the LWMS. They will ensure pathways are embedded into day‑to‑day practice, operate consistently across service areas, and support safe, effective patient flow.

The role will provide operational insight, performance reporting, and planning support to inform decision‑making, and continuous improvement. The post holder will work collaboratively with clinical and operational leads, community & clinical teams and support staff to ensure care pathways are operationally aligned, integrated across the service and sustainable. Support change management, digital pathway roll‑out, and operational integration with wider health surveillance, screening, and external services, ensuring pathway changes are implemented safely and effectively.

Please note this is a fixed term post for 23 months. Where a post only has temporary funding and an existing member of NHS Lanarkshire staff wishes to apply, this will be treated as a secondment. The employee MUST already have written agreement from their line manager to be released on a secondment before applying for the post.


In This Key Role, You Will

* Interpret current policy and professional practice relevant to the health improvement and health inequalities portfolio and where appropriate initiate developments that promote health and prevent ill health ensuring a focus on inequalities, targeting resources to local circumstances
* With local agencies and Health Improvement Department / Public Health staff, plan, coordinate and lead the development and implementation of evidence based health improvement work streams based on good practice. Responsible for ensuring these work streams are inequalities sensitive, as well as compliant with equalities legislation, reflect need and build on existing evidence to maximise the impact on health gain and reduced health inequalities.
* Analyse and interpret Health Improvement Department / Public Health and inequalities data and make recommendations for developments and changes to health improvement and inequalities work.
* Provide advice and leadership to staff and lead and support partnership working with statutory, voluntary organisations, and the community to influence and ensure commitment to deliver programmes and services which have a positive health improving effect


What You’ll Bring

* Knowledge of public health/Health Improvement field acquired through relevant degree, supplemented by specialist public health improvement knowledge, acquired through learning, extended courses and experience to Post Graduate /Masters level (SCQF level 11)
* Highly developed knowledge and understanding of the theory and practice of health improvement/public health
* Extensive experience and a proven track record of working in Health Improvement/Public Health or other health inequalities related field
* Demonstrated experience of translating and implementing relevant local and national policies to the local environment (at Board or HSCP level, or across defined communities)
* A driving licence is required; this must be a full UK/EU/EEA licence.
