We are seeking a motivatedand experienced Practice Nurse to join our welcoming clinical team. Youwill play a vital role in delivering person-centered care across a range ofnursing services, including chronic disease management, immunisations, andhealth promotion. In joining the nursing team you willwork closely with the GPs, ANPs, HCAs and be able to work autonomouslywithin our multidisciplinary team.

As a practice nurse you will playa crucial role in providing nursing care and treatment within our practice,often working alongside our GPs. Your role encompasses a wide range ofresponsibilities, from managing chronic conditions to providing healtheducation and screening.

Job responsibilities
KeyResponsibilities But Not Limited To:
Administer vaccinations andimmunisations as per practice protocols.
Cervicalcytology
Manageand monitor patients with chronic diseases (e.g., asthma, COPD, diabetes,hypertension) in line with national and local guidelines
Carry out home visits to ourvulnerable patient cohort
Support the clinical team in the management of the carehomes we oversee
Ensureaccurate and timely clinical record keeping using EMIS
Removalof sutures and clips
Contraceptionreviews
Carryout NHS health checks, new patient checks, and contraception reviews
Woundcare and dressing
Participatein infection control, safeguarding procedures, and cold chain maintenance
HealthPromotion:
Supportpatients with lifestyle advice and self-care management
Advise patients on health care, minor ailments, healthpromotion, and illness prevention.
AdministrativeDuties:
Contributeto QOF (Quality and Outcomes Framework) achievement and clinical audits
Prepare and maintain clinical rooms andequipment before, during, and after patient care activities, ensuring a cleanand safe environment for patients.
Ensure the Emergency Medication is adequately stocked andfit for purpose.
Ensure proper handling and dispatch of laboratoryspecimens, including blood and urine samples.
Maintain accurate patient records anddocumentation.
Ensure fridge temperatures are regularly monitored andmaintained.
Participate in practice meetings and contribute to ouraim to continually improve.
Support the training and mentoring of junior staffmembers.
Person Specification
Qualifications
* Registered General Nurse with current NMC registration
* Qualified & Competent in cervical cytology, immunisations, and chronic disease management
Experience
* Excellent communication and interpersonal skills, and be confident to deal with patients, families and carers.
* Ability to work independently and as part of a multidisciplinary team, using initiative within the level of competence for the position.
* Commitment to patient confidentiality and data protection.
* Accuracy and a strong desire for attention to detail
* Commitment to continuing professional development
* Working knowledge of EMIS & Docman clinical systems.
* Knowledge and understanding of GP contract QOf
* Stable career history showing clear commitment and progression
