Job overview


We are looking for a well-motivated Palliative Clinical Nurse Specialist with excellent communication, organisation and leadership skills.

Do you have experience of working in the community, supporting for people in their own homes? Do you have experience/interest in Palliative Care and are interested in working in the community?

Cheshire Wirral Partnership NHS Trust are looking to appoint a Palliative Clinical Nurse Specialist to work alongside community colleagues within the Neighbourhood Community Care Team based in Lache, Chester.

As a Palliative Clinical Nurse Specialist, you will be supported by fellow specialist practitioners including other Nurse Specialists ,Occupational Therapist, Physiotherapists and a Therapy Assistant. You will also clinically work closely with the support of the Palliative Care Consultants.

The successful candidate will receive a full supportive induction when they join. You will work closely with both your neighbourhood team, Specialist Palliative Colleagues and wider Multi-disciplinary colleagues based at the Hospice and beyond

This role is part time 22.5 hours.


Main duties of the job


The post holder will provide holistic care and support to patients with complex palliative care needs with malignant or non-malignant life limiting conditions, working autonomously and as part of a team. They will deliver care to patients registered with a West Cheshire GP in their own home. The post holder will provide advice, education and support to patients, their carers and other health care professionals.

They will also promote advanced care planning, management of complex end of life symptoms and patient and carer expectations whilst meeting the holistic care needs of the patient.

Over recent years there has been a lot development within the service and the successful candidate will need to share the teams drive to take the service forward. You also need to be passionate about delivering quality care to people receiving palliative or end of life care.

Person specification



Qualifications



Essential criteria

* Registered General Nurse
* First level degree
* Post-graduate qualification in palliative care or relevant subject or working towards
* Evidence of continued professional development
* Mentorship qualification


Desirable criteria

* Differential diagnosis or clinical examination module
* Non-medical prescribing V300 qualification
* Advanced communications skills training or prepared to work towards
* Recognised teaching qualification or prepared to work towards
* Evidence of Masters level learning or prepared to work towards


Knowledge and Expertise



Essential criteria

* High level of clinical and technical knowledge relating to palliative care.
* Teaching/assessment and presentation skills
* Exemplary written communication skills


Experience



Essential criteria

* Relevant post registration experience, at Band 5 or above in palliative care.
* Experience of multi-professional working
* Experience of teaching
* Evidence of good communications skills
* Experience of supporting and influencing change.


Desirable criteria

* Experience of audit and research
* Leadership and management experience

You must have appropriate UK professional registration.
