Carers Assessment Support - Wellbeing Team
* Monday to Friday, 9am-5pm (flexible hours available).
* Start ASAP.
* Initial 3-month contract, with potential to extend or apply for a permanent role.
* Interviews can be conducted via Teams or face-to-face.
* Role is within Doncaster with Doncaster City Council
Overview:
* The Wellbeing Team is well-established and supports adults aged 18+ with a wide range of needs.
* The team has been tasked with taking over Carers Assessments, including new assessments, regardless of whether the individual identifies as a carer.
Role Focus:
* The primary focus will be on conducting Carers Assessments and completing the associated paperwork.
* Frontline delivery of an "all age person centred" service in a locality, to help people to lead healthy, connected and active lifestyles, which allow them to remain living independently in their homes for longer.
* Ensuring that individuals have choice and control in how their support is provided.
* Customer facing delivery of early intervention and prevention solutions to reduce escalation to costly services
Ideal Candidate Profile:
* Experience working with people in a supportive, conversational capacity.
* Carers Assessment experience is highly desirable.
* Familiarity with Mosaic or similar case management systems is a plus.
* Strong interpersonal skills are prioritized over system knowledge.
* Background in community-based roles (e.g., Community Care Officer, Assessment Officer).
* Knowledge of local community resources is beneficial.
Work Environment:
* Office-based at the Civic Office for support and paperwork.
* Home visits required to understand individuals’ lives.
* Driving is essential due to city-wide coverage
