Band 6 Community Physiotherapist – Mid Cheshire Hospitals NHS Foundation Trust

Main duties of the job
* Assess, manage and provide treatment to patients with complex and/or multiple therapy needs.
* Work closely with the multidisciplinary team (MDT) and supervise therapy assistants, junior staff, apprentices and students.
* Operate across multiple sites, including other care communities, discharge to assess beds, and participate in the 7‑day urgent community response rota.
* Conduct comprehensive physiotherapy assessments, provide a therapeutic diagnosis and implement treatment plans for complex, long‑term conditions, falls, orthopaedics, mobility problems and neurological conditions.
* Monitor and evaluate treatment progress and outcomes, ensuring the effectiveness of interventions and service delivery.
* Record, maintain and store accurate, comprehensive and up‑to‑date documentation in line with legal and local requirements.
* Contribute to safeguarding, capacity assessment and informed consent, working within the legal framework and the Trust’s policies.
* Attend MDT meetings, case conferences and support discharge planning.
* Support and mentor junior staff, providing training, appraisals and supervision as required.
* Maintain HCPC and CSP registrations and comply with national and local guidelines.
* Participate in CPD, audit and research activities to continually improve practice.
Qualifications and experience
* Degree in physiotherapy.
* HCPC registered (mandatory).
* Previous NHS experience, preferably in a community setting.
* Completion of Band 5 rotation.
* Experience supervising or mentoring apprentices and students.
* Experience of audits and quality improvement initiatives.
* Good written and verbal communication skills.
* Computer literacy with Microsoft Office; accurate keyboard skills.
* Driver licence with access to own vehicle.
* Comfort with digital technology for remote working.
Benefits
Salary range: £39,959 to £48,117 per annum (pro‑rata).
