Occupational Therapist - Care Home Liaison
Job Summary
As a band 5 Occupational Therapist within the Care Home and Dementia team in TEWV you will work under the supervision of the senior occupational therapist and work in a team to manage a caseload, assessing occupational performance and delivering specialist occupational interventions, while demonstrating adherence to the Trust values at all times.
The post holder will provide evidence‑based assessment and interventions as part of the Occupational Therapy pathway and Behaviours that Challenge clinical pathway, and offer input to individuals, their families and carers in care homes.
Main Duties
* Responsible for receiving referrals for occupational therapy, and completing evidence‑based assessments and interventions to address key occupational areas of self‑care, productivity and leisure within care homes.
* Use client‑centred approaches to co‑produce negotiated occupational goals and evaluate these with service users collaboratively.
* Participate in clinical/professional and management supervision.
* Contribute to multi‑disciplinary team working.
* Contribute to service improvement and quality initiatives.
* Use the Trust electronic patient record system.
* Abide by all Trust policies and procedures.
* Respond flexibly and creatively to challenges.
Applicant must be capable of independent travel across the locality.
About Us
We are looking for a competent, confident, and enthusiastic Band 5 OT to join our team of one Specialist OT and a Band 5 OT in the Care Home and Dementia Team covering the Vale of York. The multidisciplinary team works with older people and their families and carers to deliver Occupational therapy in care homes with an emphasis on training to care home staff to help prevent relapse and hospital admission.

Job Description
The post holder will provide evidence‑based occupational therapy assessment and interventions as part of the OT Pathway linked to MHSOP Clinical pathways, and offer input to individuals, their families and carers in a Care Home setting. All team members must be committed to providing the best possible care for the people we work with and their families/carers. The ability to engage and communicate with older people with Dementia or suspected diagnosis of Dementia and their families/carers is essential. Clinical/Professional supervision is provided by the supervising Occupational Therapist. The postholder will attend OT Learning and Governance Networks to ensure fidelity to core occupational therapy delivery, enhance CPD and contribute to wider OT service developments. Demonstrate experience of delivery of occupational therapy, keenness to continually develop themselves and others, and a particular interest in this field.
Person Specification
Essential Qualifications
* Appropriate professional qualification in occupational therapy role (or pending)
* Registration with HCPC
* Willingness to undertake the Preceptorship Programme and Competency Framework within an agreed timescale
* International English Language Testing System (IELTS) level 7 for international recruits
Desirable Qualifications
* Post graduate training in relevant specialist area
Essential Experience
* Experience of working in a variety of practice areas whilst on student placements (or postgraduate)
Desirable Experience
* Experience of supporting people with mental health issues, learning disability or autism
* Experience of working in Multidisciplinary Team
Essential Knowledge
* Awareness of professional standards and their application to practice
* Understanding of the needs of service users with mental health conditions and/or learning disabilities
Desirable Knowledge
* Knowledge of evidence‑based practice in designated specialist area
* Clinical risk assessment and application in practice
