Serene Care Jersey is seeking a compassionate and reliable Support Worker to join our growing team providing person-centred care within the community.
The successful candidate will support individuals with a range of needs, promoting independence, dignity, wellbeing, and quality of life within their own homes and the wider community.
Key Responsibilities:
* Supporting individuals with daily living activities
* Assisting with personal care where required
* Supporting access to the community, appointments, and social activities
* Encouraging independence and choice
* Building positive, professional relationships with clients and families
* Following care plans and reporting any concerns appropriately
* Working collaboratively as part of a care team
Requirements:
