Are you a qualified Adults Social Worker seeking a role that makes a real difference?
Pertemps are working with a supportive and forward-thinking Adults Social Care Transition Team working across Lancashire, to find an experienced Adults Social Worker who can work collaboratively with healthcare professionals.
Adults Social Worker Role Overview:
As part of this innovative service, you will play a crucial role in providing a strengths based, outcome focussed assessment service to all adults (with the exception of adult mental health) across community and hospital settings.
Adult Social Worker Key Responsibilities:
To contribute to the delivery of strengths based social care service as outlined in national legislation and guidance and in line with county policies and procedures
To contribute to the provision of high quality assessments and support plans
To work collaboratively with health colleagues to ensure safe and timely discharges from hospital the most appropriate service and setting for the individual that actively supports their choice and independence
To promote the creative and flexible use of personal budgets to appropriately meet the needs of customers and carers
To communicate effectively with customers, carers, other agencies and professionals to support the delivery of a co-ordinated response to customer and carer needs
To effectively prioritise and manage your caseload
To keep accurate records in relation to the contact and work undertaken, using electronic record systems (Liquid Logic) and other relevant ICT systemsCandidate Requirements for the Adults Social Worker role:
A recognised social work qualification (e.g., Degree in Social Work, DipSW, or equivalent)
At least 3 years’ post-qualifying experience
Current registration with Social Work England
Experience in adult social care
Strong communication and interpersonal skills, with the ability to work effectively in a team
Knowledge of relevant legislation, policies, and procedures related to adult social careBenefits of working with Pertemps?
