About the Role: As a Social Worker in the Adults Mental Health Team, you will play a crucial role in assessing, planning, and coordinating support for adults with complex mental health needs. Working as part of a multi-disciplinary team (MDT), your responsibilities will typically include:
Undertaking comprehensive assessments of individuals' care and support needs under relevant legislation (e.g., Social Services and Well-being (Wales) Act, Mental Health Act, Mental Capacity Act).
Developing and implementing person-centred care plans, promoting recovery and independent living.
Working in partnership with service users, their families, carers, and other professionals (e.g., psychiatrists, community mental health nurses, occupational therapists).
Identifying and coordinating access to a range of community resources and services.
