We are seeking a motivated and proactive data and administration care coordinator to join our Primary Care Network (PCN) in Bury St Edmunds on a full time basis. This role is central to supporting both population health management and personalised care delivery, working with GP practices and multidisciplinary teams to ensure patients - particularly those who are frail, elderly, or living with long-term conditions, receive coordinated, high-quality care. The post holder will support the PCN manager and member practices with data analysis, reporting, care coordination, and administrative functions, contributing to service development and continuous improvement across the network. Key responsibilities include: Running and developing clinical system searches to support population health management Supporting the standardisation and implementation of service improvements Assisting with data collection, reporting, and performance monitoring Supporting MDT working and facilitating communication across teams Identifying patients who may benefit from care coordination and personalised care approaches Updating and maintaining accurate clinical records and data systems Supporting risk stratification and population health initiatives Contributing to clinical governance, audits, and quality improvement programmes Providing general administrative support for the PCN The post holder may also hold a small caseload of patients and support coordination of their care in partnership with the wider team.
