Overview
Social Worker - Specialist Adult Pathway Team (Transitions) role at Kirklees Council. 37 hours per week. Permanent position. Grade 9-11. Salary £34,434 - £44,075. Location: Huddersfield and surrounding areas.
Responsibilities
* Work with young people aged 14-25 years and their families, focusing on preparing them for adulthood.
* Undertake holistic, strength-based assessments and develop personalised support plans to meet needs and outcomes in preparation for turning 18.
* Collaborate with a range of professionals (health care providers, education, housing, etc.) to co-ordinate care and ensure seamless service delivery.
* Engage with service users, families and carers to enable independence, fulfilment, and community integration.
* Deliver high-quality, person-centred social work with experience in Adults and/or Children with disabilities; multi-disciplinary working.
* Apply knowledge of Care Act, Deprivation of Liberty Safeguards, Mental Capacity Act, SEND legislation, Personalisation, and high-quality service delivery.
* Contribute to an open, supportive team culture with managers available for advice and guidance.
Qualifications and Skills
* Experience of working with Adults and/or Children with disabilities; multidisciplinary collaboration.
* Knowledge of the Care Act, Deprivation of Liberty Safeguards, Mental Capacity Act, SEND legislation, Personalisation.
* Strong assessment, planning and documentation skills; commitment to strength-based, person-centred practice.
* Ability to coordinate with health care, education, housing and other services to deliver coordinated care.
