Key Responsibilities:
* Providing autonomous, high-quality care including assessment, diagnosis, treatment, and management of patients with undifferentiated, undiagnosed conditions.
* Managing acute presentations, minor illness, and long-term conditions within the scope of advanced practice.
* Prescribing medications safely and effectively, in line with professional and legislative requirements.
* Ordering and interpreting diagnostic tests including bloods, x-rays, ECGs, and other investigations.
* Delivering proactive health promotion and disease prevention advice.
* Supporting the achievement of QOF, Enhanced Services, and other national and local health targets.
* Working collaboratively with the multidisciplinary team to provide seamless, patient-focused care.
* Participating in audits, service improvement initiatives, and maintaining accurate clinical records.
The Ideal Candidate:
* A qualified Advanced Nurse Practitioner with MSc level training or equivalent qualifications.
* Independent Prescriber (V300) with current NMC registration.
* Proven experience working as an ANP within Primary Care, urgent care, or a similar setting.
* Confident in autonomous decision-making, assessment, and management of complex clinical presentations.
* Competent in chronic disease management, minor illness management, and health promotion.
* A strong communicator with excellent interpersonal skills and a collaborative, team-oriented approach.
* Passionate about delivering safe, effective, high-quality care and committed to ongoing professional development.
