About the Role
We have exciting opportunities for Senior Social Workers to join the Hospital Discharge Team of the Royal Borough of Windsor and Maidenhead (RBWM). These are full‑time, permanent roles with a salary of £47,831 – £52,414 per annum plus an annual retention bonus of £3,000, subject to eligibility.
About the Team
The Hospital Discharge Team works collaboratively within a multi‑disciplinary environment to provide support for people who may require adult social care services for their safe and timely discharge from hospital. The team provides high‑quality strength‑based assessments and support plans under Section 9 of the Care Act 2014, in consultation with the person, family and promoting choice and independence.
Team members engage patients and family members in the transition from hospital to home, aiming to reduce adverse events and preventable readmissions. The role requires the full range of social work tasks as required by the needs of each case and department policies, to provide the best possible service to service users.
The Role
As a Senior Social Worker you will deliver statutory services to residents with eligible care needs by conducting outcome‑focused assessments, creating tailored care plans and ensuring their effective implementation. You will manage a caseload of complex assessments, provide supervision and support to team members, and foster their professional development.
You will work flexibly with the management team, lead on key projects, chair meetings such as risk assessments and best‑interest decisions. With a focus on safeguarding vulnerable adults, meeting performance targets and operating within budgetary constraints, you will play a vital role in driving continuous improvement and making a meaningful difference in residents’ lives.
What We Are Looking For
* A recognised Social Work qualification and current registration with Social Work England.
* Significant experience in adult social care, including managing complex caseloads.
* In‑depth knowledge of relevant legislation and policies, including the Care Act, Mental Capacity Act and safeguarding processes.
* Strong leadership skills with the ability to supervise and support team members and students.
* The confidence to represent RBWM in multi‑agency settings and make person‑centred decisions.
* A proactive, flexible approach to managing change and driving service improvement.
