Role: Support Worker – City Mobile Intervention Support Team (MIST)

Salary: £32,424 per annum

Hours: 37.5 per week

Location: City of London

About Thames Reach: Thames Reach runs projects and services that help to end homelessness in and around London. We work directly with homeless people on the streets and in hostels. We work to prevent vulnerable people from becoming homeless. We help formerly homeless people get their lives back on track. We help people find work and access training and volunteering opportunities.

About this role

We are looking for a Support Worker to join the City MIST team. The Thames Reach Outreach teams operate across London and help those who are rough sleeping to find routes away from the street. The teams respond rapidly and work flexibly to reach people. Individuals are assessed to identify their support needs and support plans are created, including work to obtain identification, access health services and refer to other specialist agencies, including those offering immigration support.

Note: We will not accept applications where Artificial Intelligence (AI) has been used to assist with completing the competency questions.


Responsibilities

* Manage a caseload of clients alongside your colleagues, identifying the quickest and most appropriate route out of temporary accommodation for them, and devising a plan to achieve this.
* Establish trust-based relationships with clients who have recently been rough sleeping, supporting their permanent end to homelessness.
* Assess the support needs of new clients in a person-centred manner, using a standardised process.
* Risk assess clients dynamically and participate in risk mitigations and safeguarding work.
* Work with clients who present with a variety of support needs, including people who are fleeing domestic violence and require support and signposting to specialist services.
* Represent the team at multi-agency meetings and case conferences.


Qualifications

* The knowledge and understanding of relevant support strategies to manage a caseload of 15-20 clients.
* Excellent communication, interpersonal, and advocacy skills, proven within a context of working with vulnerable adults.
* Strong time management and prioritisation skills, and the ability to deploy these independently.
* Have a creative approach to resettlement work, and be able to work imaginatively with clients to build independence.
* The desire to work in a busy team, and collaborate with colleagues to achieve quick but sustainable outcomes for our clients.
