Overview
Part time registered nurse position within the Rapid Response team based at Swadlincote Health Centre. Scope for flexibility with hours/working patterns for the right candidate.
Responsibilities
* Manage referrals into our service and provide evidence‑based care to support patients in their own homes.
* Assess, plan, implement and evaluate individualised patient care, promoting self‑care wherever possible in an efficient and clinically effective way, while maintaining privacy and dignity.
* Provide advice and support to junior staff in the delivery of high‑quality care and actively support the senior Community Nurses/District Nurse in managing the health‑care environment.
* Work alongside general practice, therapy, district nursing colleagues and services within acute hospitals to prevent hospital admission and support discharge home.
* Collaborate with the Home Visiting service and Local Navigation Hub.
