Lead Chronic Kidney Disease Prevention Pharmacist (CVD & diabetes)
An exciting opportunity has arisen for a 12‑month fixed term role for an experienced and enthusiastic pharmacist to continue our work within integrated neighbourhood teams to optimise medicines for people living with cardio‑renal metabolic conditions across South East London. The project is a collaboration between primary and secondary care colleagues to create a person‑centred, holistic integrated model of care for people with multiple long‑term conditions to improve patient experience, change ways of working, improve outcomes and increase efficiencies.
This post is one of two roles hosted by Guy's and St Thomas' NHS Foundation Trust and King's College Hospital NHS Foundation Trust but will work across primary and secondary care sites across South East London.
This post will lead on medicines optimisation and support integration of services and pathways across the interface for renal, diabetes and cardiovascular specialties.

Job responsibilities
In liaison with the Consultant Pharmacist for Diabetes and Consultant Pharmacist, renal and transplant and under the accountability of the Chief Pharmacist, the post holder will work across boundaries with renal, diabetes and cardiovascular services in Guys and St. Thomas, Kings, Lewisham, primary care networks and South East London Integrated Care System (ICS) to:
* Be a role model and national expert in one or more of renal, cardiovascular or diabetes pharmacy field; accept case referrals and provide expert opinion on medicines issues to primary, intermediate and secondary care clinicians where appropriate, and undertake research to ensure a consistently high clinical service.
* Develop South East London (SEL) strategic intentions, and stakeholder engagement to establish clear priorities for service transformation in the field of renal, diabetes and cardiovascular disease.
* Work collaboratively with renal, diabetes cardiovascular and pharmacy departments as well as South East London Integrated Care Board medicines, primary care networks and LTC teams to enable the delivery of safe, effective and informed medicines optimisation in the context of patient care pathways.
* Use clinical and change management skills to undertake and implement care pathway redesign including provision of an overview of medicines usage and transfer of care issues across multiple patient pathways.
* Monitor, interpret and predict drug expenditure against budget.
* Work with Consultant Pharmacists and lead education and development in medicines management for the multidisciplinary team.
* Provide expert advice to GPs, clinicians and managers to ensure that national guidance and best practice underpin and medicines usage and ensure compliance with the legal framework governing medicines use to ensure patient safety.

Person Specification
Knowledge/Qualifications
* MPharm degree (or recognised equivalent).
* Postgraduate qualifications in Clinical Pharmacy Practice (PGCert, MSc or equivalent).
* Registration with the General Pharmaceutical Council (GPhC).
* Member of relevant professional groups (UKRPG, UKKA).
Skills
* Expert advanced ability to ensure safe, clinically effective and cost‑efficient use of drugs in chronic kidney disease and/or cardiovascular disease and/or diabetes.
* Hold expert advanced clinical knowledge in medicinal therapy in chronic kidney disease and/or cardiovascular disease and/or diabetes.
* Enhance the quality of patient care.
* Demonstrate knowledge and understanding of commissioning.
* Plan, implement and evaluate programmes that impact across or within departments, services and organisational boundaries.
* Effectively challenge current practice using negotiating, motivational and influencing skills.
* Able to work collaboratively with senior pharmacy, medical staff, the multidisciplinary team, management and commissioners, outside and within organisational boundaries.
* Have evidence of the ability to educate, train and evaluate pharmacy staff and other health care professionals in medicinal therapy in chronic kidney disease and/ or cardiovascular disease and/ or diabetes.
* Manage difficult and ambiguous problems.
* Develop recognition as a national and/or international expert within the speciality of clinical pharmacy.
* Proactively develop clinical pharmacy services in line with local and national objectives.
* Experience of having led a clinical pharmacy service for patients living with chronic kidney disease and/or cardiovascular disease and/or diabetes or experience of running clinics or providing expert clinical advice for people living with chronic kidney disease and/or cardiovascular disease and/or diabetes.
* Experience of managing and making appropriate referrals.
* Broad experience of clinical pharmacy services within chronic kidney disease and/or cardiovascular disease and/or diabetes.
* Experience of partnership work across the wider health economy and outside of organisational boundaries.
* Senior member of recognised pharmacy organisation (e.g. UK Renal Pharmacy Group or United Kingdom clinical pharmacy association).
