Main duties of the job
The Practice Care Coordinator will work collaboratively across alladministrative areas within the practice. This is a varied key role to includebeing the first point of contact to patients and visitors. Aswell ascommunicating with external secondary services to process and action requests.Ensuring a professional, efficient, seamless service is provided to all ourpatients.

The following are the core responsibilities of the Practice CareCoordinator. There may be on occasion, a requirement to carry out other tasks;this will be dependent upon factors such as workload and staffing levels:
Responsibility of opening/closing the practice and turning off/onsecurity alarms to ensure the practice is ready to welcome patients and safelysecure at night
Using Word, Excel, Outlook and Emis Web to facilitate the smooth operation of the practice throughefficient and effective administrative support
Monitoring multiple email inboxes and postal mail to ensure appropriateaction taken for incoming correspondence
Checking for new patient registrationforms, change of contact details and complaints
Registering of new and temporary patients
Granting online access to patients, maintaining good working knowledgeof the NHS app, Emis Web and digital procedures including electronic recordaccess and third-party consent whilst ensuring GDPR guidance is followed
Booking appointments and dealing with queries from patients, secondarycare services and external organisations
Signposting patients to services where appropriate
Processing referrals and checking the eRS worklist data
Logging and dealing with requests for reports, letters and copies ofmedical records from patients and external organisations
Taking payments for private letters, reports and medicals
Ensuring mandatory training is competed in a timely manner
Liaising with local pharmacies

Qualifications

* Good standard of general education
* GCSE Mathematics C or above
