Job Title: Continuing Healthcare Nurse Practitioner Registered Learning Disability Nurse – RNLD/ RMN/RGN Job Description: We are seeking a motivated practitioner to support the delivery of high-quality NHS Continuing Healthcare (CHC) services. The role involves undertaking and coordinating assessments and reviews, managing allocated cases, arranging care packages, and supporting eligibility and dispute processes in line with national guidance. You will work collaboratively with multidisciplinary teams, care providers, and key stakeholders to ensure safe, person-centred care, while maintaining accurate records and data systems. The post also contributes to service improvement, governance, and performance targets, and may support areas such as joint funding, Section 117, safeguarding, SEND, ABI, and transitional care. Professional registration, ongoing development, and compliance with statutory and mandatory training are essential.
