An exciting opportunity has arisen for Band 6 Nurse Practitioners to join the new Hospital at Home team in Inverness. Our Hospital at Home program aims to deliver comprehensive, patient-centred care, reducing hospital admissions and improving patient outcomes.
This service provides acute, hospital-level care to patients in their homes, for conditions that would otherwise require inpatient care. It offers a better person-centred care experience, minimizes healthcare-associated infections, and avoids long hospital stays. The initiative aligns with the Scottish Government's goal to enhance community services and prevent hospital admissions. Funding has been secured specifically for this service in Inverness.
Specialist practitioners will be supported by Advanced Nurse Practitioners and will work within an integrated team alongside AHP, Social Work, and Care at Home colleagues. This team is part of the Intermediate Care options available in Inverness.
Job Description
We are seeking compassionate and skilled nurses to join our multidisciplinary team. In this role, you will provide acute care to patients in their homes, collaborating closely with doctors, specialists, and other healthcare professionals to ensure high standards of care.
Key Responsibilities
1. Deliver hospital-level nursing care to patients in their homes.
2. Administer medications, perform diagnostic tests, and manage medical equipment.
3. Collaborate with a multidisciplinary team to develop and implement individualized care plans.
4. Monitor patient progress and adjust care plans as necessary.
5. Educate patients and their families on managing health conditions at home.
6. Maintain accurate and timely patient records.
Qualifications
* Registered Nurse (RN) with a valid nursing license in Scotland.
* Wide range of experience, particularly in Community Nursing.
* Excellent communication and interpersonal skills.
* Ability to travel to patients' homes within the designated area.
