Frailty Virtual Ward & Urgent Community Response GP - Fixed Term
GP with a Specialist Interest in Frailty Virtual Ward and Urgent Care Response. We have an exciting new role for a GP to join our interdisciplinary, integrated community team providing more acute care for patients in their own homes. This is an opportunity to expand and develop your portfolio career.
This role is sessional based and the number of sessions can be flexible dependent upon the successful applicant’s requirements. Administration time is included. The role will be a new position for the department, focusing on providing more acute care for patients safely and conveniently in their own homes as part of national frailty service design. The successful applicant will primarily work within our developing virtual ward model with a specific role supporting GPs to access the right care at the right time for their patients and to effectively utilise our growing community-based services as an alternative to hospital admission.
We work closely with the ARCH team (Acute response and rehabilitation in community and hospital), including occupational therapists, physiotherapists, nurses and multidisciplinary assistants. We also have a dedicated senior frailty pharmacist attached to the team.
Responsibilities
* To work as part of the multi-disciplinary team to support and help provide a comprehensive Geriatric Service within the Trust
* Maintain the highest clinical standards in the management of patients
* Assist with teaching, training and assessment of UCR staff and other members of the multi-disciplinary team
* Actively participate in departmental and Trust matters concerning Clinical Governance and audit
* Work with Ripon GP practices towards increased uptake of services to support Care Home residents
* Ensure active participation in continuing medical education (CME)
* Work with the Discharge Project Team to collect and interpret data on cost effectiveness and quality outcomes
HDFT provides a range of community services in Harrogate and the local area as well as across North Yorkshire. Our dedicated and experienced staff, who are based in the communities they serve, offer expertise across a variety of disciplines and work closely with primary care, acute hospitals, social care, mental health and voluntary sector providers.
We are a provider of undergraduate medical education for the University of Leeds and have a thriving research community. We have developed our Frailty education faculty, including the appointment of two frailty practice educators. This enables us to roll out frailty training with options including e-learning, face-to-face study days and more individualised EnCOP (Enhanced care of Older People) training. The successful candidate will be able to access this training and there is opportunity to contribute to the faculty.
