Working across six partner practices and five aligned care homes we employ a multidisciplinary team of Clinical Pharmacists and Pharmacy Technicians, Social Prescribers, Care Coordinators, Health Coaches, First Contact Physiotherapists and Paramedic.
We have an established, dedicated care homes team which is where this role will be placed, working alongside a Clinical Pharmacist, Paramedic, Care Coordinator and in collaboration with Geriatricians and other local health partners.
We offer hybrid and flexible working arrangements including care home visits, practice work and remote support.
Main duties of the job
Supporting medicines optimisation and proactive care for residents across care homes within the PCN.
Working within a multidisciplinary care homes team supporting quality reviews, forward care planning, medicines management, discharge reconciliation and RESPECT reviews.
Working collaboratively with GP practices, PCN Clinical Director, PCN Manager, Lead Clinical Pharmacist, Paramedic Practitioner, Development and Operational Leads, care homes and the Addenbrookes ENRICH Care Home Geriatrician Team.

Medicines reconciliation followinghospital discharge

Respond to medicines queries from carehomes
Attend care home ward rounds and MDTmeetings
Liaise with care homes, communitypharmacies and GP practices

Person Specification
Qualifications
* Registered Pharmacy Technician with the General Pharmaceutical Council (GPhC)
* Qualified and experienced vaccinator
* CPPE Primary Care Pharmacy Education Pathway (PCPEP)
Experience
* Strong communication and organisational skills
* Ability to work independently and as part of a multidisciplinary team
* Experience in pharmacy practice
* Clean and current drivers licence
* Experience in working with vulnerable patients
* Primary care experience
* Knowledge of Systm1 clinical system
