Hunter New England Health is seeking a dedicated Transition to Residential Aged Care (RAC) Coordinator to support elderly patients moving from acute hospital care into appropriate residential aged care environments. This pivotal role works closely with hospitals across the Hunter and Lower Mid-North Coast region, collaborating with clinicians, discharge planners, residential aged care providers and families to deliver safe, timely and compassionate transitions. About the Role As the Transition to RAC Coordinator, you will: Facilitate the seamless transition of elderly patients from acute care to suitable residential aged care services. Identify appropriate placement options, develop transitional care plans, and minimise delays in patient flow. Work collaboratively with multidisciplinary teams including social workers, discharge planners and case managers. Support patients with complex needs by providing accurate advice, clear communication and empathetic guidance. Build strong relationships with residential aged care providers and chair regular networking meetings to enhance partnership and information-sharing across the district. Troubleshoot barriers to placement-administrative, clinical, or operational-to ensure timely and safe outcomes. This role is ideal for a clinician with strong knowledge of aged care pathways who thrives in a dynamic environment and enjoys working across teams, services and facilities. About You You will bring: Extensive experience as a Registered Nurse or Allied Health clinician, with a background in acute care and/or community services. Strong understanding of Aged Care Assessment Program guidelines, RAC access pathways and funding arrangements. Outstanding communication, negotiation and relationship-building skills. Ability to manage competing priorities in a fast-paced setting and navigate complex patient issues including dementia, chronic disease or mental health challenges. A collaborative approach, demonstrating resilience, professionalism and a commitment to person-centred care. A current driver’s licence and willingness to travel across the region.
