This permanent role has a starting salary of £34,660 per annum based on a 36-hour working week.
We have a great opportunity to join our Connect to Community (C2C) East Mental Health/LDA team as a Senior Social Care Assistant. The C2C East Mental / LD&A Team is a friendly and supportive team with a high level of experience working within mental health and LD&A social care. We work closely with our colleagues in the longer-term teams for both MH and LD&A teams, as well as with our NHS partners such as GPs and Surrey and Borders Partnership NHS Foundation Trust.

About the Role
Connect to Community is a fast-paced, front-door environment where, as a Senior Social Care Assistant, you will have frequent direct contact with the public, both over the telephone and through visits to individuals in their own homes or care settings.
In this role, you will undertake assessments, reviews and targeted interventions in line with the Care Act, alongside taking on safeguarding responsibilities, including Section 42 enquiries. You will promote a recovery-focused, strengths-based approach, supporting individuals to achieve positive outcomes and maximise their independence. The role requires the application of sound professional judgement to manage risk, uncertainty and competing priorities effectively, whilst developing and implementing strengths-based support plans that enhance wellbeing, choice and control. You will also work collaboratively with individuals, carers, multidisciplinary professionals and service providers to ensure coordinated and effective support for vulnerable adults and their families.

Surrey has both urban and rural areas and locality based social care workers will be expected to have a valid driving licence to drive in the UK and access to a vehicle and be willing to travel across a wide geographical area. An enhanced DBS 'Disclosure and Barring Service' check for regulated activity (formerly known as CRB) and the Children's and Adults' Barred List checks will be required for this role.
