Clinical Pharmacist: Anticoagulation Service/Bromleag Care Practice
We are looking to recruit a motivated and experienced Pharmacist to work four days a week across our Bromleag Care Practice (2 days) and Anticoagulation Service (2 days). This is an excellent opportunity to be part of a dynamic, patient-focused environment, supporting our multi-disciplinary team (MDT) approach to deliver high-quality, safe, and effective care to a diverse patient population.
Anticoagulation Service: This position has become available due to the expansion of our existing team to support the growth of the Anticoagulation Service and the positive impact on stroke prevention within Bromley and Lewisham.
Bromleag Care Practice: Bromleag Care Practice seeks a dedicated and proactive Pharmacist to join our team two days a week. This role is vital to our MDT approach, enhancing care quality for residents through collaborative, patient-centred medication management and clinical support.
The base locations are:
* Anticoagulation Service: KPFH Anticoagulation Hub and clinics across Bromley and Lewisham
* Bromleag Care Practice: Beckenham Clinic, with visits to Bromley Borough Care Homes, Nursing Homes, and Residential Housing
Main duties of the job
* Assist in running community anticoagulation clinics across Bromley and Lewisham
* Work closely with the service team to achieve short and long-term objectives
* Facilitate anticoagulant prescriptions and ensure correct medication delivery
* Promote safe, effective, and economical use of anticoagulants and train healthcare staff in management
* Provide professional advice and support to care homes on safe prescribing, handling, and administration of medicines, including developing and implementing medicines management policies
* Lead on medicines management for care home patients, conducting medication assessments and reviews
* Manage face-to-face interactions with patients, care home staff, and relatives regarding medicines
* Prescribe within own competency based on patient needs
* Resolve medicines and prescription issues with healthcare professionals
* Assist patients and care home staff in managing conditions
* Lead practice repeat prescription services and handle queries from reception and patients
* Present prescribing changes to patients and relatives in line with NICE guidelines and alerts
* Advise on medication side effects and interactions

Job responsibilities
* Manage and support anticoagulant therapy in the community according to standards
* Support multidisciplinary team in anticoagulation therapies including DOACs, enoxaparin, and VKAs
* Standardize initiation and maintenance of anticoagulant therapy
* Review warfarin patients regularly, liaising with clinicians for medical opinions
* Identify and manage patients outside therapeutic INR ranges
* Screen referrals for DOAC initiation
* Provide prescribing support via clinics or telephone, following protocols
* Offer specialist advice on anticoagulant dosing
* Assess patient conditions based on INR and counsel accordingly
* Follow up with patients missing INR checks
* Support vitamin K management
* Ensure anticoagulant services align with NICE and other guidelines
* Support LMWH management
* Participate in domiciliary visits or support staff during visits
Similarly, Bromleag Care Practice Pharmacists will:
* Rationalize repeat prescriptions to prevent waste
* Provide care home reviews for residents with LTCs or complex needs
* Offer on-call pharmacist cover
* Contribute to prescribing budgets and safe, cost-effective practices
* Support medicines de-prescribing and embed best practices in care homes
* Support staff education on medicines management
Person Specification
Experience
* Experience in multidisciplinary teams and high-pressure environments
* Knowledge of NHS structures and primary care experience
* Experience in Anticoagulation Service and EMIS system
Qualifications
* Registered with GPhC
* Recent professional development evidence
* Diploma in clinical/community pharmacy

Salary: £46,148 to £53,755 per annum, pro-rata based on experience
