We have an exciting opportunity for a Lead Critical Care Outreach practitioner to join the team at the Shrewsbury and Telford NHS Trust
The Lead Critical Care Outreach Practitioner provides senior clinical leadership for the Trust-wide delivery of the Critical Care Outreach Team (CCOT) service, supporting the care of acutely ill, deteriorating and critically unwell patients across the Trust. The post holder will lead, manage, and develop the CCOT team and associated pathways across the organization and in conjunction with Critical Care Matron and Critical Care Clinical Director.
We are currently at the development stage of phase 2 of Martha's Law, the successful candidate will be integral to its implementation and development. They will also have an important role for developing the Critical Care Outreach team in line with the hospital transformation programme.
Essential to this role is knowledge and experience in Critical Care and Critical Care Outreach.
Main duties of the job
The role combines advanced clinical expertise with strategic, educational, and service development responsibilities, ensuring a high-quality, responsive, and evidence-based CCOT service.
This is a hybrid clinical and managerial role with dedicated time for both direct patient care and service development and leadership, including working autonomously within in all specialties across The Shrewsbury and Telford Hospitals NHS Trust providing expert clinical practice and leadership on a 24-hour shift rotation that covers 365 days per year including bank holidays.
This role is cross site and requires you to work at both Shrewsbury and Telford sites, being able to drive is essential.

Hospital Transformation Programme (HTP)
As part of our long-term vision to improve patient care and service delivery, our Service will relocate to The Royal Shrewsbury Hospital in 2028. This move will unite specialist services in a purpose-built, modern environment, enhancing care and team experience.
We are actively recruiting and want applicants to be aware that successful candidates will be expected to move with the Service in early 2028. The relocation will follow a formal management of change process, with full consultation and support for colleagues.
