We are offering an exciting opportunity to join the Discharge to Access (D2A) Complex Assessment team as a Discharge Facilitator based in Stoke on Trent.
You will be responsible for assessing and tracking patients requiring discharge to a D2A bed-based service in a timely manner within a non‑acute setting. You will work closely with the multidisciplinary team and external agencies to ensure a robust assessment and a safe discharge.
You will work alongside the Patient Flow Team in the D2A and community hospital settings to facilitate efficient, effective and timely utilisation of beds and resources. You will provide support to assist in the delivery of safe and effective patient transfer and tracking within D2A services while gathering and collating collateral histories and information regarding current care needs.
The service operates 24/7, 365 days per year and you are expected to work on a 5‑day per week shift pattern, which may include some weekends and bank holidays.
Responsibilities
* Gather and record information to enable accurate patient assessments to facilitate timely discharge.
* Attend meetings, share and record information, escalating any blocks or issues.
* Prioritise own workload to ensure effectiveness and efficiency in delivering objectives while working under the supervision of the Flow Team.
* Be proactive and demonstrate good organisational skills and excellent communication skills both verbally and in written correspondence.
* Work with health and social care staff and have an understanding of community Home First services along with knowledge of the Care Act 2014 and D2A principles.
