To comprehensively assess patients, gain information about their physical health, mental health, social issues, and alcohol history to develop a tailored care plan based on individual needs.
With the support and guidance of senior nurses, advise and support emergency department staff on admission criteria for patients with alcohol issues attending the department.
Build and maintain strong professional relationships with other healthcare professionals within the hospital and external agencies to meet patient needs.
Screen patients to identify those at risk from alcohol-related health and social problems (hazardous drinkers) and those whose health is being harmed by alcohol (harmful drinkers), undertaking appropriate interventions or referrals.
Conduct individual case management to provide immediate and ongoing support to patients who are unwilling or unable to engage with community services.
Attend multidisciplinary meetings to discuss referrals, frequent attenders, or patients in treatment.
Assess the risks patients pose to themselves and others, taking necessary actions within Trust safeguarding guidelines.
Educate patients and their families about alcohol-related harms and deliver brief advice.
Provide training to Trust staff on screening and brief interventions, and participate in developing and delivering educational programmes.
The Band 5 Alcohol Care Team Nurse will be a key member of the Alcohol Care Team, providing care and treatment for patients with alcohol misuse issues. Responsibilities include assessing patients, advising Trust staff on care and management of alcohol-related issues, and supporting a caseload of patients who are not engaging with community services, often leading to hospital admissions.
This role involves cross-site working, primarily based at the North Tees site, with flexible hours from Monday to Sunday, dependent on service needs.
