This post is advertised as a Band 6; however, applicants without all the necessary competencies for Band 6 are encouraged to apply. Successful candidates will be appointed under Annex 21 rules, with 70% of Band 6 pay during the first 12 months, increasing to 75% for the second 12 months or until SPQ completion.
Main Duties
This is an excellent opportunity for a registered nurse seeking to broaden their skills by providing autonomous nursing care in patients' homes as part of a multidisciplinary team. Responsibilities include palliative care, end-of-life support, managing long-term conditions, promoting self-management, and wound care. Welsh language skills are desirable but not essential; English speakers are welcome.

Qualifications
Essential: NMC registration, post-registration District Nursing qualification (BSc), experience managing diverse caseloads, and undertaking DN SPQ qualification if applicable.
Desirable: Independent prescribing, MSc in Advanced Clinical Practice, management/leadership qualifications, and experience at management level.
Experience
Essential: Significant clinical experience in patient assessment, planning, and evaluation; wide range of clinical skills.
Desirable: Competence in palliative care, chronic disease, and wound management.
Aptitude & Abilities
Essential: Excellent communication, IT literacy, ability to work independently and in teams, and engagement with the public.
Desirable: Welsh language skills and local knowledge of services.
Other Requirements
Ability to handle difficult situations, work in multidisciplinary teams, organizational skills, flexible working, and ability to travel within the locality.
