Key Responsibilities

1. To undertake tasks in delivering patient care, rehabilitation or social care as directed by service professionals, in a range of intermediate care settings and the community.
2. To progress patient treatment, following initial assessment and care planning by an appropriate service professional. To observe any changes that affects the individual and accurately communicate these back to a registered health care professional.
3. To take responsibility for the well‑being and comfort of patients. To observe an individual's ability to tolerate and manage their treatment programme. To alter, adapt or stop activity in response to the individual's ability and state of health.
4. To encourage patient compliance with treatment regimens.
5. To undertake physical measurement monitoring as directed (e.g. blood pressure, oxygen saturation, heart rate etc.) and report findings to an appropriate registered health care professional and record them accurately.
6. To transport equipment, when necessary, between patient's home and equipment store, install equipment, demonstrate its safe use and provide instructions as delegated.
7. To demonstrate patient exercise and activity as directed by therapy and nursing staff as part of their recovery. This vacancy does not offer sponsorship.

The admission avoidance team want to recruit a Band 3 Community Support Worker or Rehabilitation Support Worker with a passion for caring people in their own home and prompting independence through collaborative working. You will need to be able to identify and manage deterioration, work across inpatient and community teams and have excellent organisational skills. The Intermediate Care team is passionate about training and education and the successful candidate will be expected to provide teaching on a regular basis and will have the opportunity to receive in‑house training and complete CPD courses as appropriate.

We have formed a partnership (Ealing Community Partners) with Central and North West London NHS Trust (CNWL), The Hillingdon Hospital NHS Trust and a number of other partners to provide community services throughout Ealing.
