Join NHS Greater Glasgow and Clyde as a Band 6 Care Home Liaison Nurse within the West Dunbarton Older Adult CMHT, based at Goldenhill Resource Centre in Clydebank. Responsibilities
* Provide specialist mental health assessment, diagnosis and care planning for older adults in care homes.
* Deliver expert advice, support and education to care home staff, carers and families.
* Manage and prioritise your own caseload, ensuring effective and timely interventions.
* Liaise closely with GP, community services and multidisciplinary teams to coordinate care packages.
* Monitor and review care plans regularly to maintain independence and prevent hospital admission.
* Support hospital discharge pathways and facilitate safe, timely returns to care home settings.
* Maintain accurate records and ensure clear communication between care homes and community services.
Qualifications
* First level Registered Mental Health Nurse with significant post‑registration experience in a community setting.
* Evidence of continuing professional development and relevant post‑registration learning.
* Strong knowledge of older adult mental health conditions and therapeutic approaches.
* Excellent communication, organisational and interpersonal skills.
* Ability to work autonomously and as part of a multidisciplinary team.
* Working knowledge of relevant mental health and community care legislation.
* Full driving licence and basic IT skills.
