Overview
Qualified Social Worker - Community Team. You will be working within a Local Authority based in the Worcestershire area. The manager is looking for an experienced social worker to undertake assessments and carry a caseload. You will be arranging Package of care, Direct Payments, Extra Care Housing, Supported Living and Residential/Nursing homes.
Responsibilities
* Undertake social work assessments and manage a caseload within the Community Team.
* Arrange and coordinate care packages, Direct Payments, and housing/support options (Extra Care Housing, Supported Living, Residential/Nursing homes).
