Job Description
* The Care Coordinator ensures the highest quality care services by managing care workers and performing field-based duties.
* Assess and review individual care plans.
* Supervise, instruct and support care staff in the field.
* Maintain a valid driver’s licence and use your own vehicle for travel.
Qualifications
* Efficient, organized Care Coordinator with strong relationship‑building skills.
* Experience managing rotas to impeccable standards.
* Understanding of CQC, care plans and rotas preferred.
* NVQ Level 3 desirable.
* Previous healthcare experience beneficial.
