SouthEast Oxford Health Alliance (SEOxHA) Primary Care Network is recruiting for an experiencedparamedic to be part of the Primary Care Network multidisciplinary team working 9.30 - 17:30 on Thursday and Fridays

Thesuccessful candidate will be based in Temple Cowley Health Centre, and may berequired to occasionally work across the South East Oxford locality, consistingof three practices to provide:

1.Comprehensive health assessment and clinical review of patients.

2.A holistic approach, personalised centred care to the patients within the PCNcommunity.

3.Support to the wider multidisciplinary team


Main duties of the job

Thepost holder will provide clinical care to patients of the PCN throughsurgeries, clinics, triage clinics, on-call duties, home visits and relevantadministrative work together with such other duties as are required of thepractices.

Thisincludes the duty to record all patient data and measurements as directed bythe practices where required to maximise income under QOF, Enhanced Servicesand any other incentives which may be offered from time to time and to follow allprotocols and policies, including those relating to referrals and prescribing.

Thepost holder will work with colleagues to ensure the highest standards of carefor all patients, dealing with a wide range of presenting health needs in ageneral practice setting.

Thepost holder will also contribute to planning and decision making as part of thePCN and practices teams.

TheSEOxHA Network delivers health andwellbeing services to a population of 33,000. The local community is a rich blend of cultures and diverse backgrounds.

This is a unique opportunity todevelop the role within the Primary Care Network (PCN) and work with other teammembers to develop the PCN as a whole.

There are clinical pharmacists and a pharmacy technician, social prescribing link workers, and learning disability & care home patient coordinators in the PCN.


Job responsibilities

Responsibilities

Triage: Telephone triage of patients requesting same day appointments and otherurgent queries, prioritising the needs of patients and referring on to the GPsas necessary.

Tooffer care complementary to that offered by the GPs and other practitioners inthe Practices.

Toparticipate in acute care consultations and make differential diagnoses usingdecision-making and problem-solving skills. You will be working alongside therest of the clinical team. Patients will be allocated according to the level ofclinicians experience.

Toundertake visits to patients in their own homes and to nursing homes ifrequired during urgent/ acute episodes of care with the supervision and supportof GPs.

Toorder necessary investigations and provide treatment and care both individuallyand as part of the primary care team and through referral to other agencies.

Toscreen patients for disease risk factors and early signs of illness

Tomaintain accurate and comprehensive records with specific regards toconfidentiality.

Tomonitor and evaluate standards of care through appropriate audit, takingappropriate remedial action if standards are not achieved.

Tobe accountable for care given within Practices policies, protocols, andprocedures.

Toattend appropriate clinical update training and general and specific paramedicskills and training in order to maintain knowledge and skills.

Tobe aware of national and local targets; initiatives; in relation to ownpractice and to apply these in the clinical setting.

Torecognise professional boundaries and ensure these are maintained.

Participationin an annual individual performance review, including taking responsibility formaintaining a record of own personal and/or professional development.

Contributeto the effectiveness of the team by reflecting on own and team activities and providingsupport for other team members.

Tomaintain a professional profile and maintaining professional registration whereappropriate.


Person Specification


Experience

* Completed the 2-year consolidation of learning period as a newly qualified paramedic.
* Experience of working in primary care
* Experience using EMIS Web & Docman


Qualifications

* Fully qualified & registered Paramedic (followed & completed the Newly Qualified Paramedic Programme (NQPP)).
* ENP/ECP Minor Illness Level 6.
* Completed FCP Roadmap level 1 and prepared to complete level 2 if not done so already.
* Completed FCP Roadmap level 2.
* Independent prescriber.
