We are excited to invite applications for Specialist Familial Hypercholesterolaemia (FH) Nurses to join the Yorkshire & Humber Familial Hypercholesterolaemia Service – a dynamic, innovative, and region‑wide nurse‑led service dedicated to improving the diagnosis and management of adults and children with Familial Hypercholesterolaemia and associated lipid disorders.
About the Service
The Yorkshire & Humber FH Service provides a comprehensive tertiary‑level, nurse‑led provision across the region. The service is hosted by York & Scarborough NHS Foundation Trust, acting as the Lead Provider for a collaborative network that includes Calderdale & Huddersfield NHS Foundation Trust, Leeds Teaching Hospitals NHS Trust, and Hull University Teaching Hospitals NHS Trust.
Responsibilities
* Provide specialist nursing advice and support to patients referred into the FH pathway at the local provider site.
* Deliver a nurse‑led service to identify index cases and coordinate family screening for adults and children.
* Collaborate with local and regional multidisciplinary teams, primary care, and national partners to support evidence‑based practice, education, research and audit.
* Contribute to all aspects of the FH service, including pathway delivery, service set‑up, monitoring outcomes, quality improvement, and producing reports for stakeholders.
* Demonstrate autonomy, professional leadership and initiative to drive ongoing service development.
* Use advanced clinical, educational and management skills to ensure a seamless, patient‑centred service across organisational and professional boundaries.
Employment Details
The post holder will be employed by York and Scarborough Teaching Hospital, with Hull as their base.
