Vitalis is recruiting an experienced Social Worker to work as a Team Manager in a Community Integrated Urgent Care team in Tameside.
We are seeking a dynamic and highly motivated Team Manager to lead our Community Integrated Urgent Care team and drive service transformation in partnership with health colleagues and system partners. The Community Team includes Social Workers, Assessors, Physiotherapists, Occupational Therapists, Assistant Practitioners, and Care Assistants. IUCT offers short-term wraparound care support in people's homes for up to 72 hours.
