Overview

This role is focused on supporting people in the community with acute mental health needs who may be in crisis, and their carers (including friends and relatives) and supporters. The worker enables and assists service users to meet daily health, social care and wellbeing needs in line with personal recovery goals, and facilitates engagement with mainstream services. Mental Health Workers in this team offer a variety of psychosocial skills while working under the guidance of registered practitioners; there is also an expectation of lone working at times. The Intensive team has a shared caseload, so Mental Health Workers are not required to provide care co-ordination. The role involves working alongside an MDT for an allocated caseload of service users and organising and participating in the ongoing assessment, planning, delivering and reviewing of care to meet identified physical, mental health, social care and wellbeing needs.


Responsibilities

* Organise and participate in defined assessment methods/activities.
* Develop and review appropriate care plans and identify/organise other resources as appropriate.
* Provide defined interventions/activities as part of the care plan.
* Complete relevant paper and electronic documentation in line with the care coordinator role.
* Plan and undertake defined interventions and treatments to enable the individual to meet and develop ongoing daily living skills, to increase confidence and independence in line with agreed personal recovery plans.
* Prepare for and deliver defined self-help groups.
* Deliver therapeutic activities to improve coping skills (e.g., anger, anxiety, stress management, assertiveness or relaxation).
* Provide therapeutic activities to improve physical health (e.g., exercise programmes, diet or nutritional planning).
* Plan and undertake activities to enable individuals to meet and develop daily living skills, increase confidence and independence, and access information on housing and related matters.
* Assist with moving to new accommodation or respite care, completing forms or letters (e.g., housing benefits), and contacting employment advisors or education facilities.
* Provide emotional support focusing on motivation and encouragement.
* Provide information and advice to maintain good physical health and encourage a healthy lifestyle.
* Provide information on rights under the Mental Health Act.
* Promote continuity of care by maintaining regular contact with service users during inpatient treatment and work with other teams to facilitate early discharge.
* Work collaboratively with service users and their carers to understand and manage mental health needs in line with personal recovery plans and relapse prevention strategies.
* Organise and participate in the development of risk assessments and crisis management plans, rapid access plans, within the appropriate framework; ensure trigger points are reported to a registered practitioner and immediate or direct action is taken as needed, in line with these plans or in a crisis.

Working Arrangements

We aim to work with our staff to meet their needs and the needs of our service users. The role is in the North Somerset Intensive team, offering full-time hours. The team operates a 24/7 pattern with shift work. The North Somerset Intensive team works to promote recovery and maintains a non-exclusion policy.

Role Context

The Intensive Team provides assessment and home treatment as an alternative to hospital admission, relapse prevention, short-term therapy, and helps facilitate discharge from hospital. The team operates 24 hours per day, 7 days per week, 365 days per year; therefore, successful applicants will be expected to work a shift pattern.
