Job Description
Job Title: Support Worker (2 day staff and 2 night staff) - (3 years full experience)
Salary: Rate depending on experience
Hours: Full time positions, no part time.
Responsible to: Team Leader
Summary
To provide care and support to service users living a Supported accommodation or in the community, enabling them to become valued and participating members of the community in which they live, by achieving maximum independence.
You will support service users with all aspects of their day to day living, including social and physical activities, personal care, mobility and meal times.
Two staff to support clients during the day and two other staff to support clients during the night and carry out sleep-in care overnight.
Main Duties and Responsibilities
Support to clients with learning and physical disabilities, autism, young people going through transition from children to adulthood and mental health living in a supported accommodation or in the community.
* To understand and adhere to the philosophies, policies and principles upon which the service is based.
* To work in such a way that allows the service users to be involved in day to day and long-term decision making
* To offer support to clients enabling them to build confidence, self-esteem and an independent life style.
* To support clients in the management of their own personal care in a sensitive way – such as support around tasks at house, shopping, cleaning and cooking
* To share in, support and involve the clients in all the usual domestic activities such as cleaning, shopping and cooking when needed.
* To work with the clients in such a way that takes account of their individual, cultural and religious needs.
* To participate in the implementation of agreed individual care plans, this may include structured teaching of specialised skills as set out by the re-ablement or rehabilitation team.
* To support with booking and going with people to appointments.
* Helping individuals to get to work, college and training
* To carry out paper work such as logging daily tasks concisely and accurately where necessary on behalf of service users.
