Overview
Are you an experienced mental health professional who enjoys working autonomously? This role is based in the Gravesend locality within the Gravesend Alliance/Springfield Health PCN and aims to bridge primary care and specialist mental health services by delivering high quality mental health care in primary care settings and ensuring people’s needs are met in a timely way. There is a requirement to travel between various GP surgeries in the locality.
Responsibilities
* Provide high quality mental health care in primary care settings and work autonomously and creatively in a busy and demanding role.
* Conduct comprehensive assessments and maintain knowledge of local services; leadership experience is valuable.
* Provide supervision to band 6 Mental Health Practitioners across the locality.
* Collaborate with primary care and other organisations to support patient care and service delivery.

Person Specification
Essential
* A degree or equivalent qualification in Mental Health Nursing or other relevant healthcare profession
* State registration with the HCPC, Social Work Council or NMC
* Minimum of 4 years post-registration experience in mental health with at least 2 years in secondary care community mental health
* Experience of supervising junior staff and students
* Ability to travel independently and in a timely manner across the Primary Care Network Area
* Excellent communication and ability to work autonomously and use own initiative
* Strong organisational skills and ability to work flexibly as part of a team
Desirable
* Post-graduate qualification in a relevant area
* Experience of working in primary care
* Knowledge of Quality Improvement Programme
Skills
* Ability to communicate complex and sensitive information effectively to patients, carers/families and the multidisciplinary team
* Provide effective clinical supervision, teaching, training and assessment in clinical practice
Other requirements
* UK professional registration is required
* Disclosure and Barring Service (DBS) check required
