Wewelcome your application to join our rehab and recovery hubs across Leeds,providing therapy support with the registered therapists. Home first is theideal outcome for many patients leaving hospital, but a temporary stay in asetting that is a homely environment that can provide 24-hour care is sometimesrequired. The rehab and recovery beds do this, enabling early discharge fromhospital and reducing hospital admissions. The rehab and recovery hub servicedelivers complex care whilst empowering patients as part of a widermulti-disciplinary team. When you join us, we will work with you to developyour own personal developmental plan that supports your learning style. Weprovide training, clinical supervision and access to a peer support network, wewill enable you to develop your clinical and leadership skills. The rehab andrecovery hub service works on a shift pattern and provides a 7 day a weekservice.
Our MDTs will deliver daily evidence-based therapeuticinterventions, specifically designed to support people reach their maximumdependency state, using therapy outcome measures to demonstrate our success inachieving this. Our ethos is that the rehabilitation, recovery and reablementof service users is everybodys business.
Main duties of the job
Working in Communityrehabilitation suits people who can work independently and within the widermulti-disciplinary team. You will be resilient, motivated, and adaptable toworking in an ever-changing environment and of course, a passion for providingthe best possible care. You will have excellent communication skills andmaintain a high standard of clinical work. You will ensure compliance with ourpolicies and procedures, you will partnership work with all members of themulti-disciplinary time and wider services.
