Diabetes Specialist Nurse – Community Role
Are you interested in working with adults with diabetes in the community? The role may include clinics (face-to-face or telephone), group education sessions, and Clinical of the day.
The role is for 15 hours a week with flexibility. It is part of Herts Valleys Integrated Diabetes Service (HIDS) in South and West Hertfordshire.
Main duties of the job
We are committed to delivering a quality service, valuing staff training and CPD, regularly participating in student training, and maintaining a robust appraisal and supervisory framework. Electronic records are used throughout the Trust to enable working across multiple settings as part of an MDT, with options for remote work as part of a hybrid approach.
Responsibilities
* Work as part of the wider HID Service and integrate with community and acute settings.
* Deliver face‑to‑face and telephone clinics, group education sessions, and Clinical of the day.
* Collaborate with the MDT, support training, and participate in student supervision.
Qualifications
* Post graduate experience working within Diabetes.
* Experience of providing education.
* Excellent organisation, communication, and planning skills.
* Independent prescriber.
* Full, valid UK driving licence with access to a car for business purposes (unless disabled).

This post requires a DBS check under the Rehabilitation of Offenders Act (Exceptions Order) 1975.
Salary
£49,387 to £56,515 per annum pro rata. Includes a High Cost Area Supplement of 5% of basic salary, subject to a payment of £2,122 per annum (pro rata where applicable).
