The post of Failsafe Officer will work within the Surgery and Anaesthetics Division to develop and monitor failsafe prioritisation processes and systems to manage risk of harm to Ophthalmology patients on the holding list, and Hydroxychloroquine Service, as part of the Risk Management Strategy; to investigate incidents/complaints and make recommendations for quality improvement and safety initiatives.
Main duties of the job
Develop, Monitor and implement processes for monitoring of patient pathways in Ophthalmology to ensure that patients receive follow up review and treatment from the right person, in the right place, within 25% of their individual intended schedule for follow up.
Identify gaps, inconsistencies, errors and/ or unwarranted variation in clinical risk stratification or prioritisation of follow up, ensuring pathways are completed and accurate, with outcomes recorded and monitored and patients seen in order and a timely manner.
Liaise with secretaires, clinicians, service managers, waiting list staff and outpatients where patients are not booked within required timescales to agree and implement actions required to ensure full compliance.
Ensure all patient events are appropriately recorded, accurately and timely on the Patient Administration EPR system (Cerner)
Undertake full validation and correction of all patient pathways to support achievement of 18 weeks referral to treatment and retinal pathway knowledge is essential to enable the safe management of all patience.
Must have and Maintain advanced IT skills to enable effective record monitoring, report extraction and ability to use Cerner (Trust EPR), Medisite, Microsoft Excel, Word, Power point, Desktop Publisher and Outlook. Also have knowledge of Zeiss and Heidelberg.

MAIN DUTIES

Develop audit systems and tracking reports to identify errors that result in patients being lost to follow up.
Provide regular reports on activity and tracking compliance to clinicians and managers for HCQ Service.
Work with secretaries, clinicians, nursing staff and managers on service demands and available capacity, implementing any changes required to maintain a fully compliant service.
Provide day to day support to the Ophthalmology Deputy Directorate Manager and Admin Manager managing the holding lists, Hydroxychloroquine service and other services looking at requirement for capacity clinics, setting up and booking of clinics.

Investigate, solve incidents/complaints with patients being lost to follow up in accordance with Trust Policies.
Form good working relationships with key personnel to influence change and proactively manage capacity.
Communicate information, sometimes of a sensitive nature with Department Manager and nursing staff, external staff, clerical, managerial and admin staff.

Essental Ophthalmology Experoence & Knowledge
Essential criteria
* Ophthalmology Experience
* Extensive NHS experience, Ophthalmology knowledge of all sub specialities within service and have excellent working knowledge of Cerner (Trust EPR) have comprehensive 18 weeks knowledge, tracking and validation experience to high level.
* 5 GCSE including English at ‘C’ grade or above or equivalent and Business admin qualification
* Knowledge of systems and processes in particular knowledge of booking systems and Clinic Management
* Ability to cope under pressure and meet challenging deadlines- Examples
Desirable criteria
* Degree in relevant subject or demonstrable equivalent experience in Ophthalmology
* Ophthalmology Knowledge and have good problem-solving skills
* Able to provide reasonable flexibility in terms of hours worked
