What you will do
* Manage and lead the Transition to Adulthood Team, guiding them to achieve the best outcomes for young people aged 16 to 25 as they move into adulthood.
* Oversee and support a team of eight Personal Advisors, ensuring high standards of care and compliance with relevant legislation and policies.
* Develop and implement strategies and plans to aid young people in successfully transitioning from local care services to independent living.
* Collaborate effectively with a range of professionals and stakeholders, including education and health services, to provide comprehensive support plans.
* Monitor and evaluate the team’s performance, providing constructive feedback and guidance to promote continuous improvement.
