Are you passionate about clinical pharmacy and ready to make a strategic impact on renal and urology patient care? Barts Health NHS Trust is recruiting for a Lead Clinical Pharmacist to drive high-quality, patient-focused services and medicines optimisation across our network.


Responsibilities

* Provide advanced clinical pharmacy leadership for renal and urology wards, dialysis units, and outpatient clinics.
* Lead a dedicated pharmacy team, ensuring continuity and excellence in service delivery.
* Develop, implement, and review evidence-based prescribing and formulary guidance.
* Optimise medicines safety and cost-effective prescribing practices, minimising adverse reactions and improving patient outcomes.
* Collaborate with multidisciplinary medical, nursing, and allied health teams to support effective patient flow and care.
* Manage the unit's drug budget; design and deliver medication-related cost improvement plans.
* Oversee medicines reconciliation, development of Patient Group Directions and Shared Care Guidelines.
* Guide safe transfer of prescribing and monitoring between secondary care and other settings.
* Support strategic service development in line with Trust, departmental, and ICs vision and plans.


Scope

The post holder will carry out the full range of duties as the Lead Clinical Pharmacist – Renal and Urology to deliver patient focused, quality care across the network covering renal and urology wards, dialysis units, renal assessment units and outpatient clinics. The post holder would be expected to lead the clinical pharmacy service in the Renal and Urology services in providing responsible, safe and cost-effective prescribing within the networks to: Optimise patient outcomes and Minimise adverse reactions. The post also helps to deliver the broader Medicines Optimisation and Governance Strategy across the Trust. These outcomes will be achieved primarily through leadership of clinical staff, and the publication and implementation of guidelines and procedures to encourage best practice. The post holder will support the Associate Director of Pharmacy, Deputy Head of Pharmacy & Clinical Services Manager and Pharmacists within the team to manage the professional, clinical and technical input of pharmacy into the Renal and Urology directorate.


Work Patterns

The post holder might be required to work across the Trust at any time throughout the duration of his/her contract, which may entail travel and working at a different hospital site.
