The post holder will provide consultancy on issues relating to the social care needs of forensic clients of the Community Forensic Learning Disability Service (CFLDS), offering skilled advice and guidance to staff within the CFLDS and the wider social care community, in accordance with legislation.
Responsibilities include:
1. Providing a skilled social work service to individuals with a Learning Disability.
2. Working collaboratively with service users and carers to assess and identify their needs.
3. Contributing to the development of future service initiatives.
4. Assisting in the assessment of suitable placements.
5. Participating in multidisciplinary teams to develop care plans aligned with risk assessments and needs.
6. Coordinating care with other professionals in forensic services.
7. Producing reports reflecting the needs of individual service users and risk issues.
8. Engaging with service users, families, and carers to ensure awareness of their rights.
9. Partnering to meet social care needs arising from offending behaviour.
10. Utilising the Electronic Patient Record system for data input.
11. Promoting anti-discriminatory practices within society and policies.
12. Ensuring service users have access to advocacy services.
13. Advocating for the specialist support team to embed advocacy in practice.
14. Ensuring safeguarding policies for children and vulnerable adults are in place.
15. Implementing MAPPA, Victim Liaison Services, Domestic Violence, and National Probation policies.
16. Maintaining contact with statutory and third-sector agencies to inform assessments.
17. Participating in MDT groups for service development.
18. Contributing to research and training for professionals.
