Job overview

The post holder will be responsible for line management of the ACP team within the south Community ACP team which includes delivering yearly appraisal and the Clinical Academic Support Panel (CASP) with support from the trust’s Professional and Medical ACP leads when needed. They will investigate and assist in resolving complaints and incidents related to advanced clinical practice. In addition, they will act as a professional role model for all members of the department.

The Lead ACP will also practice as an independent, autonomous, accountable practitioner working on their own initiative and taking a lead role in clinical decision making, working collaboratively with the wider multi-professional team.

They will fulfil a senior role in the clinical team with the autonomy to initiate action based on their interpretation of policies/guidelines, seeking advice if required. This will include providing expert advice and guidance to junior colleagues on differential diagnosis and treatment plans.

The successful candidate will have extensive experience as an ACP within the community setting.

Main duties of the job

The individual will work closely with consultant practitioner, ACP leads and the senior leadership team. They will take appropriate responsibility for training and mentoring the ACP team, ensuring that clinical governance requirements are met, in accordance with the trust/national standards for example - evidenced clinical supervision, and competence sign off. The role may incorporate facilitating inter-professional education of doctors in training, nursing, and allied health professional when appropriate.

In addition, they will support the Consultant Practitioner and senior leadership team in relation to Advanced Clinical Practice and work closely with the STH Professional Lead for Advanced Clinical Practice

The role will be structured around the four core functions of advanced practice:

1. Expert advanced level practice (with a minimum clinical focus of 60%)
2. Professional leadership and consultancy
3. Education, training and development of self and others
4. Practice and service development, research, and evaluation

Education

Essential criteria

5. Relevant Masters’ degree in Advanced Practice or recognised qualification via the supported portfolio route from HEE Centre for Advancing Practice
6. Registered Nurse with Valid NMC registration or Health Care Professional and registered with HCPC with relevant clinical experience
7. Advanced Physical Assessment module, Non-Medical Prescribing (if part of proposed role)
8. Advanced life support qualification (ALS/ILS)
9. Evidence of relevant continuous professional development
10. Recognised Leadership Award/qualification or equivalent experience

Desirable criteria

11. Recognised Teaching & Assessment Qualification or equivalent experience

Skills and Knowledge

Essential criteria

18. In depth knowledge of pathophysiology of common conditions seen in the speciality and their management
19. Able to analyse and synthesise patient information to formulate appropriate diagnoses and management plans
20. Can prioritise and manage own workload and support junior personnel in managing theirs when faced with conflicting pressures
21. High standard of written and verbal communication - able to present and receive complex information and communicate effectively with all grades of staff and patients / families
22. Excellent presentation skills with the ability to communicate effectively with students/ colleagues/ external providers
23. Ability to manage self and maintain professional conduct in difficult and challenging situations
24. Effective leadership, negotiation and reasoning skills with the ability to influence, coach and motivate others
25. Knowledge of strategic vision and understanding of national and Trust agendas for advanced level practice
26. Knowledge and experience of bespoke advanced practice appraisal processes e.g., Practitioner E-Portfolio & CASP

27. Knowledge of various methods/theory of education delivery e.g., technology enhanced learning, simulation

Experience

Essential criteria

28. Significant relevant experience as qualified ACP
29. Significant experience within the speciality of community care and experience working within community setting
30. Must be able to evidence and demonstrate experience of ability to act as an independent, senior decision maker in a clinical team using established curriculum and competencies
31. Evidence of portfolio of practice and sign off of competence of knowledge, skills and experience across all 4 pillars of advanced practice
32. Experience of undertaking service improvement activity
33. Evidence of contribution to audit and research activity
34. Experience of training, supervising and assessing competence of junior colleagues (medical staff / ACP's / NMAHPs)
35. Evidence of adopting strategic priorities into practice
36. Clinical / managerial leadership experience, including line management of staff, e.g., sickness/absence, recruitment/selection
37. Experience and understanding of clinical governance in a practice setting and of dealing with complaints and managing / investigating clinical or other incidents

38. Evidence of leading on implementing strategic priorities into practice
39. Experience of planning, developing and co-ordinating training material, courses, and activity for others
40. Experience of leading on change management or service improvement activity
