We have an exciting opportunity for a Band 7 Diabetes Specialist Nurse to join our award‑winning proactive Hounslow Community Diabetes Service.

We are seeking an experienced Diabetes Specialist Nurse who has the passion and skills for improving the care of people with Type 2 Diabetes (and Type 1 Diabetes in special circumstances). The role would suit someone who is highly motivated with a positive approach to high quality care delivery with excellent communication and leadership skills to work in partnership with our community and primary care colleagues.

Experience of working with virtual technology to support a flexible service in complex Diabetes clinics and group settings would be welcome. The role includes provision of respectful evidence‑based care in nurse‑led step‑up and step‑down clinics service; and joint care for housebound patients and patients with special needs to enable supported self‑management skills for patients and their carers in partnership with the community team.

The service supports a consultant‑led multidisciplinary team clinic.

Diabetes Specialist Dietitians work closely with the Diabetes Specialist Nurses and oversee the delivery of our X‑PERT Structured Education Programmes for patients with Type 2 Diabetes.

The post is based at the Heart of Hounslow Centre for Health.


Main duties of the job

* The post holder will work effectively as a member of the multidisciplinary team which includes patients, Specialist Diabetes Nurses, Diabetes Specialist Dieticians, Diabetes Consultant, Primary Care colleagues and community teams, to improve holistic health outcomes for patients with Diabetes.
* Provide specialist care, advice and support through direct booking Primary Care hubs (PCN) and jointly support the development of integrated working.
* Provide joint practice‑based clinic sessions to optimise care planning and prescribing decisions for complex patients in GP practices.
* Triage and audit referrals to the service and provide relevant service and evidence‑based feedback to referrers to provide timely care to patients.

The role is full time (37.5 hrs per week); however, part time applications will also be considered in line with service needs.


Person Specification

The person specification below outlines the criteria against which your application will be assessed.


Qualifications

* Registered Nurse
* Relevant Nursing or Health Degree
* Mentor / teaching qualification
* Clinical supervision training and experience
* Independent nurse prescribing qualification or willing to working towards this


Experience

* Minimum 2 years recent post registration Diabetes experience
* Experience in nurse‑led management of long‑term conditions
* Experience in implementing protocols and clinical guidelines
* Experience in quality initiatives such as clinical benchmarking
* At least 2 years recent primary and community nursing experience
* Audit skills
* Team leader experience


Skills and Abilities

* Able to demonstrate a commitment to the WLT values
* Clinical leadership skills for autonomous practice and coaching others
* Negotiation and conflict management skills
* Change‑management skills and ability to support patients to change lifestyle
* Excellent communication skills, both written and verbal
* Ability to communicate difficult messages to patients, families and health care colleagues
* Teaching and mentorship experience in a clinical setting to multidisciplinary team members
* IT skills - Microsoft and clinical systems
* Self‑directed and organisational skills to support smooth running of the service
* Line management skills and /or training


Knowledge

* Competency in the management of patients with diabetes
* Knowledge of health promotion and behavioural change strategies
* Knowledge of clinical governance issues in community and primary care
* Awareness of local and national health policy
* Awareness of national and local public health issues
