Responsibilities
- Support a busy service delivering safe and timely discharges across a 200‑bed pathway.
- Hold a reduced caseload of complex work and support the wider team to ensure assessments (14–28 days) and reviews (6–12 weeks) are completed within expected timeframes.
- Act as a key point of support for less experienced staff, offering guidance around decision‑making, prioritisation and best practice.
- Work within a light‑touch triage system, ensuring cases are appropriately managed and workflows remain efficient and person‑centred.
- Mentor others, maintain high standards, and be a reliable source of knowledge within a fast‑paced environment.
- Work closely with a multidisciplinary network, contributing to effective discharge planning and ensuring individuals are supported to return home safely with the right care in place.
- Collaborate within a service that values structure, supportive leadership and collaboration, shaping outcomes for team and people.
- Participate in hybrid working, with flexibility to work from home up to two days per week.
Qualifications
- Qualified Social Worker and registered with Social Work England.
- Strong experience within adult social care.
- Confidence supporting others in practice.
- Statutory role requiring the above credentials.
