Are you an experienced Nursing Assistant/Health Care Support Worker that is passionate about keeping patients in the comfort of their own home to avoid hospital admissions, or supporting early discharge? Then this is the role for you.
Hospital at Home is a community-based service providing care at home within a multi-disciplinary team (MDT). We are a service in South Manchester as part of Manchester Community Response (MCR), working alongside other services such as Crisis response, Discharge to Assess, and IV. We take referrals from both hospital and community and provide input for clinical, therapy, and social needs, allowing patients to stay in their own home. As a part of MCR, although predominantly working within Hospital at Home, there is an expectation to work across the floor to support other teams we work alongside.
We will support you with specialist and bespoke training to reach your potential and excel in your chosen career path. You will receive one-to-one clinical support, professional development, and guidance.
Roles and Responsibilities
* Deliver clinical care for patients in their own home, working without direct supervision, with immediate access to a qualified staff member for advice and direction on care as required.
* Undertake venepuncture, obtain ECGs, and complete observations and document accordingly.
* Prioritise your own caseload of work and be responsible for tasks delegated from a registered nurse.
* Communicate effectively with patients, relatives, carers, and other healthcare practitioners, internal and external to the team, ensuring any concerns are listened to and relevant actions are taken.
* Act in a manner that illustrates respect for privacy, dignity and confidentiality.
Requirements
* Full UK driving licence with access to a vehicle. You will be travelling across the South Manchester locality.
* Commitment to a seven-day service working between the hours of 08:00–22:00 on a rota basis.
* Capability to work without direct supervision.
* Experience with venepuncture and ECG acquisition.
